"Failed leadership at Veterans Administration put patients at risk, watchdog reports" from Chicago Tribune

A 150-page report was released yesterday that disclosed details about “failed leadership at multiple levels” at the Veterans Administration. This was during the time of the Obama administration when the VA was headed by Secretary David Shulkin, who also resumes the position now. There were key patient safety issues at one of the VA centers in Washington, D.C. as well as many other systematic issues within the system.
